Identifying Heart Failure Patients upon Admission

  • The first step in identifying patients with heart failure is reviewing the hospital discharge form or referral form.
  • If the candidate is on a Diuretic, Beta-blocker or Ace inhibitor or have a diagnosis of heart failure they are placed automatically on the protocol. 
  • We then either identify them as high or low risk using the NYHA classification. 
  • The baseline intake form is implemented and the doctor's order is written.

 

Project Funded by the McGregor Foundation
In conjunction with the UHCMC. 

McGregor is the pilot facility for this innovative project. 

The project is projected to expand to several other facilities in the area soon. 

The mission of the project is to decrease hospitalizations, smooth transitions betweens all levels of care, increase the knowledge level of participants and increase the delivery of care for people with the diagnosis of heart failure.
